OzPol Community Care provides a range of government funded and full cost recovery services toAustralian (Aus), ATSI, European (Eur) & Eastern European (EE) communities in Queensland. OzPols service delivery and philosophy are driven by the quality standards encapsulated in the Accreditation and Community Care Common Standards. OzPols focus on genuine community engagement for consumers and reduction of social isolation. OzPol address their clients aged care needs by focusing on improving access, and establishing, organizing and promoting services and facilities that foster the consumers mental, physical andsocial well-being. In service support areas that OzPol do not provide, OzPol fosters links with specialist providers and sub-contractors to ensure integrated services and optimal outcomes forconsumers. OzPol offers the community a suite of Government funded aged care services including: HomeCare level 1-4, allied health, social support, domestic assistance, transport and centre-based day respite, community visitor scheme, DVA, Transition Care and residential care. Through provisionof services we actively encourage clients to focus on maintaining independence and living in theirown homes for as long as possible.
