Adelaide Primary Health Network Limited is a large-scale charity established in 2012. Their main activity is registered as senior services primarily serving adults - aged 65 and over aboriginal and torres strait islander people gay, lesbian, bisexual, transgender or intersex persons migrants, refugees or asylum seekers people from a culturally and linguistically diverse background financially disadvantaged people people at risk of homelessness/ people experiencing homelessness people with chronic illness (including terminal illness) people with disabilities pre/post release offenders and/or their families unemployed persons victims of crime (including family violence) victims of disaster females males.

Adelaide PHN was established by the Commonwealth to reshape the delivery of primary health care across the Adelaide region. Our activities are aimed at improving health outcomes in the community, keeping people well and out of Hospital. We have key membership groups of community members who provide input on strategic plans and feedback to the organization. We work in collaboration with other health providers in both the community and tertiary sector to improve patient pathways and models of care. We support all primary health care professionals, assisting with access to education and training and ensuring that there is key information about chronic disease states, different models or care and referral pathways and resources.
